How to Apply For Care

A house call is an ideal alternative for those who cannot go to a doctor’s office for a traditional visit. We commonly provide house calls to people who are either too sick or too weak to get out of bed. Patients can schedule a house call for a non-emergency or a non-life threatening situation. Note-For an emergency situation, a patient should call 911 immediately.

To apply for care through the HousecallsNY.org service, a patient should complete our registration and medical release forms and return them to our office. Click here to print out the forms or contact our office at (212) 740-6400 and we will send them to you.

Once those forms are completed, a patient or a family member can set up an appointment by calling our 24 hour scheduling service. House call appointments can be arranged immediately or can be scheduled at whatever time is convenient for the patient.

A medical professional will visit patients on a regular basis as often as needed or medically necessary. House calls can generally be made within 24 to 48 hours of the request from the patient, family, nurse or caregiver. Our scheduling department will call and confirm appointments within the 24 hours before each visit. Your visit will be scheduled for either 9:00 AM - 12:00 noon or 12:00 noon - 5:00 PM. Due to circumstances beyond our control, such as traffic and patient emergencies, it is difficult to provide exact times.

For patients insured by the government, we will submit forms for payment, for others, payment can be made at the time services are performed. Following the visit, patients may then submit their claims to their insurance companies for reimbursement. We will supply all the necessary billing codes and paperwork for you to send to your insurance company. If you require further help, our staff can assist you.
