How to Refer a Patient
At HousecallsNY.org, our goal is to make in home
medical treatment a simple process for the patient using the service
and the healthcare professional referring the patient. Before making
a referral, a health care professional should determine if a patient
is suitable for the HousecallsNY.org service. Following are the
types of patients that are able to utilize this service:
Non-mobile or geriatric patients
Patients who are too weak to get out of bed
Patients who are unable to leave their home
Patients that are disabled

If a patient qualifies, they can sign up for
our service by simply filling out our registration and medical release
forms and returning them to our office. Click

Once those forms are completed, a patient or
a family member can set up an appointment by calling our 24 hour
scheduling service. House call appointments can be arranged immediately
or can be scheduled at whatever time is convenient for the patient.
A doctor will visit patients on a regular basis,
as often as needed or medically necessary. House calls can generally
be made within 24 to 48 hours of the request from the patient, family,
nurse or caregiver. Our scheduling department will call and confirm
appointments within the 24 hours before each visit. Due to circumstances
beyond our control, such as traffic and patient emergencies, it
is difficult to provide exact times.
Payment can be made at the time services are
performed. Following the visit, patients may then submit claims
to their insurance companies for reimbursement. We will supply all
the necessary billing codes and paperwork for patients to send to
their insurance company. If additional help is required, our bilingual
staff is available to assist patients coordinate paperwork with
other healthcare providers.
